H&F Community Support & Resilience Fund 2026/27
Grant Monitoring Form DRAFT

Please note, this monitoring form may be subject to change and the final version will be provided to successful grant recipients in early 2027.

Click here to return to the main application process page.

Thank you for the work you have delivered through the H&F Community Support and Resilience Fund. This form helps us understand what the funding has achieved, who it has reached and what you have learned along the way. We know that not every organisation will collect information in exactly the same way, so please give your best estimates where exact figures aren’t available and add any context that you think would be helpful.

Please provide equality and demographic information that you routinely and appropriately collect, in line with the agreed monitoring guidance. We do not require small organisations to collect sensitive demographic information solely for this fund where doing so would be disproportionate or inappropriate. If you have any questions or feel any sections will take a disproportionate time to complete, please contact [email protected] to discuss.
